Client named above represents, warrants and agrees that he/she is in good physical condition and that he/she has no disability, impairment, or ailment that will prevent him/her health, safety, comfort, or physical condition should he/she engage in active or passive exercise.  he/she understands that his/her participation in the program is voluntary, and that he/she is free to deny consent to participate in the program at any time that he/she is free to deny consent to participate in the program at any time that he/she desires. HE/SHE ACKNOWLEDGES THAT THE INSTRUCTOR HAS NEITHER MADE CLAIMS AS TO MEDICAL RESULTS NOR SUGGESTED THAT HE/SHE SHOULD STOP EXERCISING IMMEDIATELY IF HE/SHE SHOULD STOP EXERCISING IMMEDIATELY IF HE/SHE DETECTS ANY ABNORMAL DISCOMFORT OF ANY SORT DURING THE COURSE OF THE EXERCISE PROGRAM.

By signing this document, the client acknowledges that he/she has been informed of the need to obtain a physician's examination and approval prior to beginning this exercise program. With full appreciation of the various hazards and risks associated with an exercise program. The client desires to participate. He/She expressly accepts and assumes full responsibility for his/her health and any resultant injury or mishap that give up any right that he/she may have against the facility, the instructor, or any personas involved with this program or testing procedures.  HE/SHE DECLARES UNDER PENALTY OR PERJURY UNDER THE LAWS OF THE STATE OF CALIFORNIA THAT THE FOREGOING IS TRUE AND CORRECT.
